Programme Benefits

​Health Up! is currently open to Singapore Citizens and PRs who are aged 40 and above, and living in Tampines.

As a participant of Health Up!, you will:

  • Gain a deeper understanding of your current health and fitness levels
  • Receive a Health Action Plan Personalised for You (HAPPY) that outlines screening and lifestyle recommendations tailored to your current habits and needs
  • Learn how to set and achieve realistic health and fitness goals
  • Receive updates on the latest health and social events in Tampines
  • Receive a Gym-in-a-Bag and other incentives (Limited to the first 250 sign-ups)

Key Components of your Health Up! Journey

Health Screening

Many medical conditions are "silent" in the early stages – you may have the condition but still feel completely well. Regular screening can help you detect medical conditions early, allowing for earlier treatment and management.


Conditions that you can get screened for (if eligible for the subsidised screening):

  • Diabetes
  • Hypertension
  • Hyperlipidaemia
  • Obesity
  • Colon cancer
  • [For females only] Cervical and breast cancers


Six-Week Wellness Programme

Week 1: Pre-Programme Assessment

An experiential session at Active Health Lab (Our Tampines Hub) to help you understand your body composition, fitness and health status.

Weeks 2 to 5: Workshops with Active Health Coach

For participants with no medical conditions:

Virtual Workshops

For participants with conditions such as diabetes, hypertension and/or obesity

Week 2:  Learn how to use the Active Health Play Mat to incorporate simple and fun activities into your daily activities.

Weeks 3 to 5:  We will touch on different physical activity components including

  • cardiovascular fitness;
  • balance and muscular fitness; and
  • mobility and flexibility.


Week 2:  Virtual health coaching session

Learn about the concept of vision and goal setting, and how to plan an exercise regime that best suit your needs.

Weeks 3 to 5:  In-person workshops at Active Health Lab (Our Tampines Hub) 

Understand the principles of frequency, intensity, time, and type of aerobic, strength and flexibility exercises suitable for your condition. Our coaches will guide you to ensure a safe and enjoyable exercise experience.

 

Week 6:  Post-Programme Assessment

Our Active Health Coach will conduct an assessment to check on your progress and provide an update on pre-participation data. You will also have an opportunity to share feedback on the journey and set long term goals for your health.

Your journey does not end here!   

To encourage you on your lifelong journey towards better health, your Well Being Coordinator will connect you to a wide variety of nutrition and health programmes, as well as sports interest groups run by SportSG, HPB, and Our Tampines Hub.
