The SingHealth Regional Health System (RHS) recognises that our population can be empowered to keep well, get well and age well in their communities and homes.  Across our three Campuses – SGH, CGH and SKH – we have adopted a One SingHealth, multi-site approach that allows us to share resources and learn from one another, together with SingHealth Community Hospitals and SingHealth Polyclinics.

Nurses from SGH, CGH and SKH are stationed at Community Nurse Posts in the heartlands to help seniors with chronic conditions manage their conditions.

We welcome referrals from medical professionals and social service organisations.

EMPOWER Study

Our study aims to improve patient empowerment through the EMPOWER app. If you are aged 40 and above, clinically diagnosed with Type 2 diabetes, and are physically able to exercise, we need YOU in our study!
