Enhancing Advance Care Planning, Geriatric Care and End of Life Care in the Eastern Region
Under the programme, geriatricians, nurses and allied health professionals from CGH partner nursing homes in the East to:
Develop professional capabilities in Geriatric and End-of-Life care
Provide training on how to facilitate Advanced Care Planning discussions with patients and their family members
Provide training on how to support End-of-Life care preferences for nursing home residents
Over 100 nurses from these nursing homes have been trained to date:
Salvation Army Peacehaven Nursing Home
Moral Home for the Aged Sick
Lions Home for the Elders
NTUC Health Chai Chee Nursing Home
All Saints Home (Tampines)
