The elderly’s wishes are simple: to have easy access to healthcare when they need it, and to live independently and confidently in the community, close to their loved ones. At SingHealth, we work closely with community health and social care partners to help the elderly age in place. At the same time, we partner these organisations to build professional capabilities in community care. 

                          

Community Nursing

Nurses provide health screening, falls prevention screening, chronic disease education, and post-discharge care to elderly residents.  

CareLine 

A 24/7 hotline operated by a team of Client Service Associates that provides health and social support to vulnerable elderly. 

                         

Advance Care Planning

Sharing your wishes in advance helps your loved ones understand what treatment you would prefer in a medical crisis.

Holistic care to improve the well-being of patients with advanced Parkinson's Disease and their caregivers.

Training: Geriatric and Palliative Care 

Collaboration between Changi General Hospital and nursing homes in the East to develop professional capabilities in geriatric and end-of-life care.
