Bringing nursing care closer to you
Community Nursing at SingHealth Regional Health System aims to take person-centred care beyond hospital walls and into the community.
To achieve this, nurses from Singapore General Hospital, Changi General Hospital and Sengkang General Hospital work closely with community partners to help elderly residents to keep well, get well and age well near or in their homes.
The nurses provide services such as: health screening and coaching, falls and frailty assessment, chronic disease education, post-discharge care and coordination, home visits, caregiver training, and home palliative care.
