Learning By Design, Not Chance: Approaches in Healthcare Simulation to Foster Innovation and Adaptability

Despite the implementation of numerous patient safety initiatives in today's healthcare, the occurence of avoidable healthcare morbidity and mortality remains high. To achieve sustainable safety in the delivery of care, healthcare organisations must seek innovative ways that combine continuous learning with high performance and service.
Central to this is the creation of a psychologically safe environment where the focus is on continuous learning rather than apportioning blame and the goal is problem solving and distribution of learning. Measuring success as the organisation’s ability to adapt to the variance of its systems through collective learning.
Achieving this requires the creation of an infrastructure that breaks down silos, integrating governance, education and service delivery to ensure learning by design rather than by chance.
I will describe our approach to applying simulation in different ways, which allows us to harness the passion and curiosity of our staff to innovate and adapt. Supporting them to translate learning into new ways of behaving in the clinical setting. Changing their work as done and giving them ownership of achieving their aspirations, engendering a feeling that things can change and motivating them to strive to continually improve.
