Palliative care is a person-centered approach that supports people with serious life-limiting illnesses, such as advanced cancer or organ failure. It focuses on alleviating suffering and improving quality of life by addressing physical, emotional and spiritual needs of patients and their families.
SCH provides inpatient hospice palliative care services (IHPCS) across Outram Community Hospital (OCH) and Sengkang Community Hospital (SKCH). SCH IHPCS accepts referrals of patients from acute hospitals, as well as community home hospice providers.
Our Team
Our dedicated multidisciplinary palliative care team works closely to bring comfort and dignity to patients and families during this important stage of life. Our team consists of doctors, nurses, medical social workers, physiotherapists, occupational therapists, dietitians, and speech therapists.
We aim to provide:
- Person-centred care aligned with their values and wishes
- Physical symptom management (e.g. pain, breathlessness)
- Emotional and spiritual support to patients and families
- Legacy work for patients to left precious gifts and memories behind for their loved ones
- Grief and bereavement support to families before and after the loss of a loved one
- Support to fulfil patients' preferred place of care and death
