Needy Patients

As our population ages, we see more elderly people facing the challenges of chronic illnesses in our community hospitals. The majority of patients admitted to our hospitals are elderly aged above 60, and about 96% require government subsidies and other financial assistance after means testing

Even with subsidies, many patients struggle with the additional costs that come with extended healthcare journeys. The SCH Health Fund enables patients in need to receive essential medical treatments, rehabilitation services, and necessary supplies like diapers and mobility aids. By easing financial pressures, your donation allows patients to focus entirely on what matters most – their recovery and journey home to loved ones.

Financially Needy Patients

Even with subsidies, essentials items such as diapers, mobility aids, and medical supplies place significant strain on families.

The Fund provides targeted, accountable assistance, allowing patients to focus on recovery rather than financial stress.

Social Prescribing

Recovery requires more than clinical care – it requires connection, purpose, and belonging. Our social prescribing programmes link patients to meaningful community activities, from digital literacy sessions to tailored engagement initiatives, supporting long-term wellbeing, confidence, and independence.

Palliative Care

Every life deserves comfort and dignity. Our palliative teams provide pain management, emotional support, and guidance for families navigating serious illnesses. The Fund strengthens programmes that ease both physical and emotional burdens, helping patients and families find comfort when it matters most

Dementia Care

Dementia cases are rising rapidly. Care must extend beyond the hospital.

The Funds supports

  • Cognitive therapy programmes
  • Caregiver support initiatives
  • Dementia-friendly environments
  • Innovative programmes such as Mindful Mart, where patients earn reward points through therapy to redeem daily essentials, fostering autonomy and engagement

These initiatives enhance quality of life while reducing caregiver stress.

Psychosocial Wellbeing

Healing extends beyond physical recovery.

The Fund supports therapeutic environments – peaceful gardens, quiet reflection spaces, and family-friendly areas – fostering emotional resilience alongside medical care.

Research, Innovation & Education

Sustainable community care requires continuous advancement.

The Fund supports research, professional development, and innovation that improve care delivery, strengthen workforce capability, and advance patient outcomes – today and for future generations.
