Realising the Needs for Living Well
It is estimated that more than 150,000 people in Singapore would be diagnosed with dementia by 2030. We want to take a preventive approach towards dementia, and introduce cognitive intervention programmes so that rehabilitation goes beyond physical functional abilities.

At SCH, we care for our patients with dementia through

• Reminiscence activities to encourage multi-sensory engagement, improve communication, relieve negative emotions and restore a sense of belonging
• Creating a dementia-friendly environment that is accessible, comfortable and safeInter-professional education to equip healthcare professionals with the skills to better care for patients with dementia
• Caregiver support and community partnerships to ensure continuity of care post-discharge
• Ongoing research to guide staff development and cognitive stimulation activities for patients
