As our population ages, we see more elderly people facing the challenges of chronic illnesses in our community hospitals. The majority of patients admitted to our hospitals are elderly aged above 60, and about 96% require government subsidies and other financial assistance after means testing.

Even with subsidies, many patients struggle with the additional costs that come with extended healthcare journeys. The SCH Health Fund enables patients in need to receive essential medical treatments, rehabilitation services, and necessary supplies like diapers and mobility aids. By easing financial pressures, your donation allows patients to focus entirely on what matters most – their recovery and journey home to loved ones.

Palliative Care

Every life should be embraced with dignity, comfort and meaning. At SCH, our palliative care team walks alongside patients during their most vulnerable moments, providing expert pain management, emotional support, and compassionate care that honours each person's journey. We focus on enhancing quality of life, helping patients find comfort and acceptance while maintaining their dignity.

The SCH Health Fund supports comprehensive initiatives that bring comfort to patients' final days and vital support to their caregivers. From programmes that ease physical discomfort and emotional distress, to services that help families navigate this difficult journey together. We also recognise the power of meaningful moments through our Wish Fulfilment Programme - arranging outings to memorable places, enjoying comfort food, or spending precious time with loved ones.

Dementia Care

It is estimated that more than 150,000 people in Singapore would be diagnosed with dementia by 2030. Through the SCH Health Fund, we are taking a comprehensive approach that complements existing hospital services to enhance the quality of life for patients with dementia, focusing on preventive measures and cognitive intervention programmes.

The Fund supports innovative care solutions like our assistive robots for cognitive stimulation and the Mindful Mart programme, where patients earn reward points through therapy sessions to redeem daily essentials. These initiatives have shown significant improvements in patient engagement and independence in dementia patients.

At SCH, we provide holistic dementia care through technological solutions, reminiscence activities, dementia-friendly environments, professional education, caregiver support, and ongoing research. The Fund amplifies these efforts by enabling programme expansion and technology adoption, creating a multiplier effect on existing hospital services.

Social Prescribing

Healthcare goes beyond medical and rehabilitation. Social determinants of health, such as an individual's housing environment, literacy, food security, social support and access to health and social care resources, can heavily influence a person's health outcomes. Through Social Prescribing, we engage each patient as a unique individual, filling their hospital stay with meaningful social activities that reflect their community life. Our Wellbeing Coordinators teach older patients digital skills, including smartphone use for staying connected and enjoying entertainment through video streaming apps.
