The SCH Health Fund is the fundraising arm of SingHealth Community Hospitals (SCH). It bridges the gap for patients who have exhausted all other sources of financial aid, giving them access to essential treatment and care services.

SingHealth Community Hospitals (SCH) comprises Sengkang Community Hospital, Outram Community Hospital, and the upcoming Eastern Community Hospital, which will open around 2029-2030. We care for patients who need sub-acute, rehabilitative, dementia and palliative care – the majority being seniors above 60 years of age.
Our focus is to give patients and their families the confidence to live well after scharge. While acute hospitals focus on treatment and surgery, SCH helps patients bridge the transition from hospital to home. Working closely with acute hospitals and community partners, we ensure patients are equipped to get well, stay well and continue living well in the community.

Rehabilitation Care
Recovery does not only happen in therapy rooms – it happens every time a patient takes another step towards independence. At SCH, rehabilitation is part of daily life: patients exercise in gyms, practise walking on garden paths, and rediscover joy through activities such as art or horticulture therapy. With every movement and every moment of encouragement, patients grow stronger and more ready to return home.

Dementia Care
For patients with dementia, a safe and calming environment can make all the difference. Our dementia wards are specially designed to support memory and orientation, using reminiscence therapy, doll therapy, and group activities. Behind this is a team trained in dementia care, ensuring patients feel secure, respected, and meaningfully engaged – while families have peace of mind that their loved ones are well cared for.

Palliative Care
At life’s most delicate stage, dignity and comfort matter most. Our palliative care team walks alongside patients and families, focusing on what truly matters to them. From legacy projects that allow patients to leave cherished memories behind, to memorial services that help families find closure, we honour each life with compassion and respect. With initiatives like compassionate discharge and community admissions, patients are supported to live and leave with dignity.

Social Prescribing
Good health is about more than medicine. Through social prescribing – pioneered by SCH in Singapore – we address social and emotional needs that affect recovery. Our Wellbeing Coordinators listen to patients’ stories, connect them with meaningful activities, and link them to community resources after discharge. For some, this means joining an art session; for others, learning how to use WhatsApp to stay connected with loved ones. Each small step helps patients not just recover, but continue living well at home.
