[OCH] SSN Karuppannan Prabhagaran 
In our combined Renal and Rehabilitation ward, we support patients on their journey to independence. I take on various roles and hold many responsibilities such as nursing duties, supporting mobility training with the rehab team, educating patients and caregivers on managing their conditions, and more. Overall, I aim to create a supportive and goal-oriented environment that will motivate patients, while ensuring their safety, comfort and dignity through the recovery process.

I hope that as they recover, they can regain not just their abilities but their confidence as well. In the near future, I hope to further develop my expertise in sub-acute care by deepening my knowledge in inpatient rehabilitation and advanced clinical practices. With the foundation I have built through pursuing advanced certifications, I would like to focus on improving patient outcomes through evidence-based care. 

I also hope to take on leadership roles within the team, contributing to mentorship and inspiring the next generations of nurses to come!


[SKCH] SN Tan Hui Ting

After graduation, I was assigned to the palliative ward, where my role focuses on managing patients' symptoms, particularly pain control, to ensure their comfort during life's final journey whilst maintaining their quality of life. One particularly memorable experience involved an elderly couple in our ward—the husband receiving palliative care whilst his wife was undergoing rehabilitation. Despite her own health challenges, she remained steadfastly by his side.

During a poignant conversation as his condition deteriorated, she confided in me about her heartbreak, yet expressed her unwavering belief in their eternal love. Their story of enduring devotion, especially as they navigated life's final chapter together, reinforced my understanding that our role transcends mere medical care—it's about honouring the profound human connections that give life its deepest meaning. Looking to the future, I aspire to extend palliative care services into patients' homes. This transition would enable us to provide more personalised and compassionate care, better supporting both patients and their families in familiar, comfortable surroundings.
