• Practicums will be held at SingHealth Community Hospitals (Outram Community Hospital or Sengkang Community Hospital). During the practicum, all learners will be assigned to staff and are expected to take part in multi-disciplinary discussions. Learners will also participate in classroom tutorials where they will get to learn the techniques of facilitation, patient social health assessment, recommended solutions to challenges and cross-share their learnings and applications with their mentors and other learners.

​A / P Lee Kheng Hock

Prof Lee Kheng Hock is a Senior Consultant at the Department of Family Medicine and Continuing Care at Singapore General Hospital (SGH) as well as a Director at the Office of Community Engagement and Education, SingHealth Community Hospitals (SCH).

He is an advocate for social prescribing and has developed SCH's Social Prescribing Programme that won the Best International Social Prescribing Scheme at the Social Prescribing Network Awards 2021.

Ms Irnina​
Irnina specialises in the domains of social work and community development, and has over 9 years of experience in both sectors, both local and overseas.

In NP's School of Humanities and Social Sciences, she currently teaches and coaches students from the Diploma in Community Development, towards crafting out their future professional career aspirations and pathways in the social and community services setting
