The Khoo Teck Puat Centre for Population Health was officially launched under SingHealth, following a generous gift from the Estate of Khoo Teck Puat. This contribution represents a significant investment in advancing population health efforts across Singapore, with SingHealth Community Hospitals (SCH) playing a central role in the Community Health domain.

SCH focuses on supporting patients through critical transitions in their health journey, including recovery after acute care, rehabilitation, and end-of-life care. Its community hospitals primarily serve older adults with complex medical and social needs. Care delivery is anchored in a person-centred approach that goes beyond medical treatment to address physical, emotional, and social wellbeing. Close collaboration with acute hospitals and community partners ensures continuity of care beyond discharge.
 


The gift will enable SCH to strengthen initiatives across six interconnected areas: support for financially needy patients, social prescribing, dementia care, palliative care, psychosocial wellbeing, and capability-building through research and education. These efforts reflect the integrated and holistic nature of care within community hospital settings.

One area expected to see significant impact is social prescribing. Many patients face challenges such as social isolation or limited access to community resources, which can hinder recovery and long-term health outcomes. By identifying social risks early and connecting patients to meaningful activities and community support during their hospital stay, social prescribing promotes smoother transitions and sustained recovery after discharge.

The contribution will also enhance dementia care and caregiver support. Platforms that capture patients’ life stories will enable more personalised and dignified care, while digital tools will provide caregivers with practical guidance following hospitalisation. In addition, selected spaces within SCH will be redesigned to support psychosocial healing, recognising the role of care environments in emotional wellbeing and recovery.
