Many countries are actively working on creating innovative health and social care delivery models or systems in response to the growing complexity of population health issues. This track offers a critical platform for researchers, policymakers, and practitioners to join, collaborate, and catalyse transformative change and integration in health and social care. Recognising our shared goal of achieving health equity and wellbeing across all age groups in urban settings, we acknowledge the multifaceted nature of factors influencing population health outcomes. The central theme of our track revolves around pioneering innovations that bridge the gap between research, policy, and practice with a specific focus on forward-looking initiatives to support the wellbeing of vulnerable populations. By bringing together a diverse range of stakeholders, we seek to foster interdisciplinary dialogue and exchange cutting-edge insights that drive evidence-based decision-making and actionable solutions. Through an inclusive lens, we explore how emerging technologies, community-driven initiatives, and practice innovations can synergise to enhance access, equity, quality of care leading to positive health outcomes, while tackling the underlying causes of health disparities. We welcome abstract submissions that have the potential to contribute to the development of best practice guidelines for implementing transformative approaches in health and social care delivery. Commentaries and insight papers on the role of leadership in the co-creation of cross-disciplinary policies and programmes that bridge health-social outcomes may be considered.
