• Patient/Caregiver/dependant(s) must have activated personal notifications on Health Buddy to be reminded to submit health data prior to appointment. 
• There is no automatic review of your submission. Your clinic will inform you if you need to submit data prior to appointment. 
• Please ensure that you choose the correct ‘switch profile’s records when sending health data to SingHealth. You will be prompted to verify during Singpass login again for confirmation.
  
• Selected *modules will be able to send a total of 3,30 or 40 records at single submission in a day. If you wish to submit more than the limit, you may do so in another submission as there is no limit to the number of submissions per day.

• Please install Health Buddy and submit Health Data on one device phone. All your submissions within Share Document will not be reflected if you are logging in/viewing from another device. Do note that submissions within Share Document will show an overview description of submission. There's no action required to open the card in Health Buddy in Share Document's Sent Health Data storage. 
  
   

• You can use this function to send your self-input data securely. There is no risk of misplacing your health data for your upcoming consultation. 
• The data received may be used for SingHealth’s aggregated research analysis. 
• If you are a caregiver, both you and the patient (your dependant) will be able to track/view your submitted Health Data digitally for ease-of-communication during your clinic visit consultation.

​There are 3 different prompts which you can verify before sending Health Data to SingHealth

1. If you're a caregiver, both you and the patient (your dependant) can click 'switch profile' button to submit health records accordingly.
2. Health Buddy will preselect the no. of latest records (as explained above), you can confirm the selection of your records to submit depending on their adherence.
3. Patient/caregiver/dependant(s) login Singpass to confirm the record(s) that they will be submitting from period DD MMM YY to DD MMM YY.
   
   

​Yes, you will need to login via Singpass once you have confirmed your selection to submit Health Data.
