Health Screening

​Healthier SG Screening is the national health screening programme by Health Promotion Board. To complement SFL, SingHealth Regional Health System provides health screening to help residents living in Eastern Singapore find out if they have or are at-risk of developing chronic conditions such as diabetes, high blood pressure and high blood cholesterol.

Early detection of chronic conditions or risk factors will improve efforts to prevent or delay the onset of chronic diseases.

After screening, residents who require follow-up care will be referred to SingHealth Community Health Posts (CHPs) for early intervention, which includes regular coaching and monitoring of lifestyle habits by healthcare professionals. The CHPs are located at Community Centres, Residents' Committee (RC) Centres and Senior Activity Centres.

Note: For Healthier SG, we may collect and use your data to (i) facilitate your enrolment with eligible primary care providers; (ii) review your health status and outcomes to tailor strategies and plans; (iii) provide services to support your health needs; and (iv) monitor the effectiveness and outcome of our efforts.

We may also disclose your data to third parties who need your information in order to facilitate your participation in Healthier SG or provide services to support your health needs. We may share your data with family doctors and community partners where necessary to support your care and health outcomes.

The data may include identifiable data, including data on your care components and health status.
