Community Health Centre

SingHealth operates two Community Health Centres (CHCs) in Tiong Bahru and Tampines to support GPs in managing patients with chronic conditions.

The CHCs are helmed by experienced nurses and allied health professionals who provide health education, dietetic services, physiotherapy, Diabetic Retinopathy Photography and Diabetic Foot Screening. Services offered may vary depending on the needs of the GPs and residents in the vicinity.

Please note that the CHCs only accept referrals endorsed by general practitioners (GPs). Patients on the Community Health Assist Scheme (CHAS)  may enjoy subsidies at CHCs.
