Value-Driven Care

Value-Driven Care is an approach to improving healthcare by measuring and comparing care outcomes for specific medical conditions. By using data to understand what works well and where improvement are needed, we can continuously enhance the quality of care we provide.

Our philosophy prioritises improving clinical outcomes, with cost-effectiveness and healthcare sustainability following as natural consequences of delivering better care.

Since launching in 2017, VDC has expanded to cover 65 medical conditions across SingHealth's hospitals, national centres, community hospitals and polyclinics. This allows healthcare teams to learn from one another, identify best practices, and improve care for patients across the healthcare cluster.

How We Are Scaling VDCs Across SingHealth:

(a)  Cross-setting VDC: following the patient, not just the episode

Cross-setting VDC follows patients across different care settings, covering prevention, long-term management of chronic conditions, treatment during acute illness, and recovery or rehabilitation. This gives us a fuller picture of outcomes across the whole care journey.

SingHealth's first cross-setting VDC tracked patients with Diabetes receiving care across hospitals and polyclinics. Building on this, a second initiative is exploring a similar approach for patients with Asthma.

(b)  Tools that make VDC stick

Individual Clinician Report (ICR)

The Individual Clinician Report shows clinicians how their patients’ outcomes have changed over time and how their results compare with those of their peers. Seeing this information clearly helps clinicians identify what is working well and where care can be improved.

In use at SGH since 2022 across 15 VDC conditions, the results speak for themselves. Following rollout, hysterectomy CQI rose from 81.5% to 90.5%, including an improvement of blood transfusion avoidance from 91.5% to 96.6%.
