SCH Palliative Care

Palliative care is a person-centred approach to caring for people with serious life-limiting illnesses, such as advanced cancers and end-stage organ failures. It involves providing care for our patients' physical, psychosocial, emotional and spiritual needs, in order to relieve suffering and improve quality of life.

SCH provides inpatient hospice palliative care services (IHPCS) with a total of 48 inpatient beds across Outram Community Hospital (OCH) and Sengkang Community Hospital (SKCH). SCH IHPCS accepts referrals of patients from acute hospitals, as well as community home hospice providers.



Our multidisciplinary palliative care team consists of doctors, nurses, medical social workers, physiotherapists, occupational therapists, dietitians, and speech therapists. Members of the team work closely together to support patients and their families through their inpatient stay. We also provide support for families and loved ones of patients during their end-of-life journeys and post-bereavement support.

  • Psychosocial, emotional and spiritual support, legacy work
  • Grief and bereavement support
  • Financial counselling, advanced care planning
  • Support to fulfil their wishes of preferred place of care and death
