About Social Prescribing

Social Prescribing is a means of connecting patients to a range of non-clinical services in the community and improve their health or wellbeing. In addition to “medical prescription”, a “social prescription” is needed to complete the care plan. In this post-conference workshop, we will explore what it means to family physicians and how we can apply this to our area of work.

Objectives of the Workshop

Knowledge of the concept of social prescribing and its relevance to family medicine and social prescribing.
Apply the extended consultation method to co-create a care plan.
Adopt a motivational interview approach to social prescribing.
