Social Prescribing is a means of connecting patients to a range of non-clinical services in the community and improve their health or wellbeing. In addition to “medical prescription”, a “social prescription” is needed to complete the care plan. In this post-conference workshop, we will explore what it means to family physicians and how we can apply this to our area of work.
