Screen for Life (SFL) is the national health screening programme by Health Promotion Board. To complement SFL, SingHealth Regional Health System provides health screening to help residents living in Eastern Singapore find out if they have or are at-risk of developing chronic conditions such as diabetes, high blood pressure and high blood cholesterol.
Early detection of chronic conditions or risk factors will improve efforts to prevent or delay the onset of chronic diseases.
After screening, residents who require follow-up care will be referred to SingHealth Community Health Posts (CHPs) for early intervention, which includes regular coaching and monitoring of lifestyle habits by healthcare professionals. The CHPs are located at Community Centres, Residents' Committee (RC) Centres and Senior Activity Centres.
