​1.  What is Health Up!?

Health Up! is an inter-agency collaboration led by SingHealth to promote health among residents in the East through health screening and the adoption of healthy lifestyles. The programme aims to empower members and their families to adopt healthier lifestyle habits, and participate in age-appropriate health screening and physical activities to attain better health and wellness.

Health Up! brings together the expertise of healthcare providers and community partners, including SingHealth Polyclinics, Changi General Hospital, local GP partners, People’s Association, Health Promotion Board, and Sport Singapore. Leveraging the strengths of our community partners, we hope to ensure the long-term sustainability of healthy lifestyle habits.

2.  Who can take part in Health Up?

Health Up! is currently open to all residents aged 40 and above and living in Eastern Singapore, who are Singaporeans or Permanent Residents. Please indicate your interest by filling in this online form (15 minutes to complete).   

3.   What happens after I join Health Up?

Upon successful enrolment, our Wellbeing Coordinator will review your health status based on your responses in the Onboarding Questionnaire, and develop a personalised Health Action Plan for you. If you are eligible for health screening, you will go for screening with local family doctors like our GP partners and polyclinics, or at community screening events, to assess your current health status.

You will also take part in a tailored health and wellness programme to learn how to set fitness goals and develop an exercise routine suitable for your fitness level and lifestyle.

To encourage you to sustain your healthy lifestyle habits, you will be introduced to a wide range of community-based exercise programmes and interest groups offered by the PA, SportSG, and HPB based on their health status, needs and goals.

Throughout the journey, your Wellbeing Coordinator will check in with you periodically to guide and follow up on your progress. 
 
4.  Do I have to pay?

There are no enrolment or membership fees for eligible residents. However, you will need to pay for the subsidised health screening and follow-up consultations with your family doctor, if you have abnormal health screening results.
