​The SingHealth Delivering On Target (DOT) programme right-sites patients with stable chronic conditions from the Specialist Outpatient Clinics (SOC) at Singapore General Hospital and National Heart Centre Singapore to partner GPs.

1. Type 2 Diabetes Mellitus
2. Ischaemic Heart Disease
3. Congestive Cardiac Failure
4. Chronic Kidney Disease
5. Asthma
6. Orthopaedics
7. Anti-coagulation
8. Urology (Benign Prostatic Hyperplasia, Overactive Bladder, Elevated PSA, Lower Urinary Tract Infection)
9. Rheumatology (Gout, ANA Positive)

Benefits for Patients

1. Convenience of seeing a GP near their homes
2. Consultation charge at participating GP clinics capped at maximum $32
3. Fast track referral to SOCs as subsidised patients if their conditions deteriorate and require specialist care
4. Subsidised medication with delivery option to patients' preferred address from SGH or NHCS pharmacy
5. Subsidised diagnostic laboratory and allied health tests
6. Follow-up support by SingHealth DOT Right-Siting officers (RSOs) who act as the main liaison between patients, GPs, allied health professionals and doctors.
