  5. General Medicine - Hyperlipidemia, Hypertension
  6. Geriatric Medicine - Fall Prevention, Incontinence, Cognitive Impairment/Dementia, Malnutrition, Functional Decline, Depression
  7. Orthopaedics - Osteoporosis, Lumbar/Cervical Spondylosis, Osteoarthritis (joints)
  8. Renal - Chronic Kidney Disease, Kidney Donor
  9. Respiratory - Asthma
  10. Rheumatology - Gout, Serology positive, Sjogren's Syndrome, Soft Tissue Rheumatism

  • Follow-up support by RSOs who act as the main liaison between patients, GPs, and specialists
