Person-Centred Care – From Theory to Practice

Person-centred care is an emerging concept and key component in service transformation to meet the evolving healthcare landscape. It can transform the relationship between patients and their healthcare professionals through shifting from a paternalistic care approach towards a collaborative and holistic approach that recognises people as active partners in their own healthcare. This conference track will feature a collection of person-centred care theoretical approaches and models that have been developed and applied to practice both internationally and locally. Our panel of expert speakers will focus their presentations on (1) conceptualising and spreading the adoption of person-centred care in SingHealth, (2) understanding and implementing Local Area Coordination—a person-centric model of care that seeks to elicit and tap on a person’s motivations and personal strengths to meet his/her care goals—and (3) applying Healthy Conversations Skills—a person-centred communication tool that equips healthcare professionals to empower people to improve their own health and promote positive behaviour change. Participants will gain a deeper understanding and appreciation of person-centred care theories and acquire practical knowledge and skills to implement person-centred care models effectively.
