Improving population health requires engagement of diverse stakeholders to co-create solutions to address health issues and enhance population wellbeing. This track will gather population health experts, health and social care providers, as well as patients and caregivers to provide a comprehensive perspective on the opportunities and challenges in implementing population health initiatives, including Healthier SG.
