SSN Bautista Juliet De Guzman from Ward 56 highlights how simple attentiveness can make a real impact on preventing harm.
While routinely reviewing a patient’s medical history and investigation results in preparation for an elective surgical procedure, Senior Staff Nurse (SSN) Bautista Juliet De Guzman from Ward 56, noticed jarring discrepancy in the affected area shown in the patient’s medical history and the intended surgical site. Recognising the potential risks of an incorrect surgical site, SSN Juliet clarified her doubts with the doctor and escalated the matter to the medical team. Her keen attention to detail and timely intervention allowed the team to make the appropriate adjustments before the surgery began, preventing any potential patient safety incidents from occurring.

Another moment that illustrates her exceptional vigilance was during medication rounds where she spotted a duplicate prescription. Instead of simply administering the dose as prescribed, SSN Juliet immediately raised her concerns to the doctor. Thanks to her swift action, the duplicate order was corrected, mitigating the risk of an overdose.
These incidents reflect the personal and professional standards that SSN Juliet holds herself to. Despite the long and tiring hours at work, she remains vigilant, conscientiously checking for inconsistencies and clarifying doubts quickly. In her words, “Every nurse must uphold the highest standards of care, and take responsibility for protecting patients from preventable harm”.
While it can feel daunting to speak up to those in positions of greater authority and experience, SSN Juliet would like to encourage her colleagues to pause and remember what is truly important – patient safety. “By raising concerns early and speaking up when we notice anything out of the ordinary, we can support one another to keep our patients safe.”
