Understanding Virtual Care Services

Telehealth does not replace in-person clinic visits, it works alongside physical sessions in a hybrid model tailored to an individual’s needs and preferences.

Patients with stable conditions can benefit from occasional in-person assessments to ensure holistic care. This balanced approach means patients with less complex medical needs may opt for virtual services, while those who have more complex care needs will use in-person services.

Whether virtual or in-person, the services are delivered by the same trusted healthcare professionals at our polyclinics. We maintain identical clinical standards across all platforms, supported by secured digital platforms and following national healthcare guidelines. Types of Video Consultation Services at SHP     

1. General Medical Conditions

✔ Same-day doctor consultations and expedited medication delivery for patients with non-emergency or minor conditions, such as cough, cold and gastroenteritis.

2. Chronic Conditions Follow-up

✔ Ongoing care for existing patients for chronic conditions such as diabetes, hypertension, and preventative care.

• Virtual check-ins to monitor progress and propose adjustment of treatment plans

3. Family Planning

✔ Ongoing care for existing patients of SHP for oral contraceptive pill follow-up.  

• Guidance on contraception and side effects
• Discuss sensitive topics from home
• Discreet medication delivery or self-collection service

4. Telehealth Monitoring (Tech-Enabled Care [PTEC])

✔ Remote care and monitoring of blood pressure and blood glucose with virtual medication adjustment (tele-titration).

• Secure transmission of patient’s vital signs to care team
• Early intervention if any concerning trends are detected

Emphasising the importance of telehealth monitoring, Dr Scott Heng, Resident Physician at Pasir Ris Polyclinic explains, "Remote telehealth monitoring lets us practice healthcare more proactively. We are no longer limited to relying on snapshots from clinic visits. Home glucose readings or blood pressure data show us patterns we would otherwise miss. This means earlier adjustments, fewer emergencies, and more tailored care."

Preparing for Your Video Consultation

SHP’s VCs are conducted through ‘Zoom’, a mobile and desktop video conferencing application. Meeting details such as date, time and the meeting link will be provided through SMS and the SingHealth Health Buddy mobile application after one has successfully made an appointment.
