RIRS is a minimally invasive surgical procedure used for the treatment of stones or other conditions (including tumours) within the kidney.
It involves passing a flexible ureteroscope (Figure 1) through the urethra, bladder and up the ureter into the kidney, without the need for an incision on the body. Within the kidney, laser fibres or stone retrieval baskets can be introduced through the ureteroscope for fragmentation and retrieval of stones respectively.

How is RIRS performed?
The procedure is usually performed under general anaesthesia (GA). A thin and flexible ureteroscope is inserted through the urethra, bladder and ureter to reach the kidney. The high-definition camera within the scope allows visualisation of the stone, which is then either fragmented or turned into fine dust using a laser (usually Holmium:YAG laser). The fragments are then suctioned out of the kidney or removed using a stone retrieval basket.
What can patients expect after RIRS?
After RIRS, a ureteral stent is usually left in place between the kidney and bladder. This is a thin, soft tube approximately 20-30 cm long that serves to:
The stent is usually left in place for 1-2 weeks and removed in a short outpatient procedure – either via flexible cystoscopy under local anaesthesia, or by pulling a string attached to the stent.

These symptoms are common, expected, tend to be mild and can usually be managed with oral medications.

Singapore General Hospital (SGH) is a major tertiary referral centre, with over 50,000 outpatient attendances annually and around 1,500-1,800 surgeries performed a year in the Department of Urology. The Department of Urology offers holistic evaluation of patients with suspected urolithiasis, as well as the entire array of stone treatments including Extracorporeal Shockwave Lithotripsy (ESWL), RIRS and Percutaneous Nephrolithotomy (PCNL). SGH performs a high volume of stone surgeries, likely the highest within the Singaporean context, with around 400 cases of RIRS and 100 cases of PCNL and Endoscopic Combined Intrarenal Surgery (ECIRS) a year.
Complex stones are also managed, including complete staghorn stones and stones in patients with an abnormal anatomy including transplant kidneys or in patients with previous urinary reconstruction (e.g., ileal conduit).
There have been several recent advances that have revolutionised the role of RIRS and significantly increased its effectiveness.
Flexible and Navigable Suction Access Sheath (FANS)
In recent years, RIRS has been performed with the aid of an access sheath, which is first placed into the kidney and allows the scope to be passed in and out of the kidney.
The Flexible and Navigable Suction Ureteric Access Sheath (FANS) represents one of the most significant recent advances in RIRS technology. FANS includes a sheath that is placed into the kidney with two key additional features:

SGH has since incorporated FANS into all RIRS procedures resulting in high efficiency of stone clearance and lower risk of complications.
