​"The multidisciplinary Transitional Home Care (THC) team meets weekly to discuss the progress of patients with complex conditions and develop discharge care plans, based on their health and social needs.

As a pharmacist, my role is to monitor their progress and make recommendations to optimise their medication therapy. It is almost like doing a jigsaw puzzle as we piece together a personalised care plan for each patient.

We see tremendous improvement in patients' experience of care after discharge when they receive appropriate care at home, well supported by their family members."
