From the population health perspective, end-of-life care involves understanding and addressing the social, economic, cultural, and environmental factors that influence end-of-life experiences. Beyond symptom management and medical interventions, the goal is to promote compassionate, person-centred care for individuals and communities at the end of life. This track will discuss emerging themes from this field, including identifying barriers to equitable access to palliative and supportive care resources; exploring culturally tailored approaches to advance care planning, decision-making, and bereavement support; strengthening community networks and build capacity to provide practical, emotional, and spiritual support to individuals and families facing the end of life; and embracing innovative models of care delivery or interdisciplinary team-based approaches to improve care coordination and continuity.
