Social Prescribing

​Discharging Patients Into a Community of Care
Healthcare goes beyond just medicine and rehabilitation. Social determinants of health, such as an individual's housing environment, literacy, food security, social support and access to health and social care, are social-environmental factors that can heavily influence a person's health.

SingHealth Community Hospitals has introduced Social Prescribing in our wards, allowing us to engage the patient as a unique individual and spend time on person-centred conversations. We then fill patients' stay with a wide range of social activities that are reflective of their lively community back home.

The next step is to connect patients to community services to instil in them the confidence to live an active and healthy life, supported by us and the community.

A pioneer in Social Prescribing in Singapore, we:

  • Introduced the Social Prescribing programme in our hospitals in 2019, adapting it to teach elderly patients advanced smart phone use so they can stay connected during the COVID-19 pandemic

  • Are a member of the Global Social Prescribing Alliance

  • Collaborated with World Health Organization (WHO) on producing a Social Prescribing Toolkit and co-produced WHO global online training course

  • Are a winner of the Best International Social Prescribing Scheme at the Social Prescribing Network Awards 2021

  • Organised Asia-Pacific's inaugural Social Prescribing Conference in 2022 and launched the Singapore Community of Practice in Social Prescribing in 2023
