Medical Social Services
SCH Medical Social Services, comprising our team of Medical Social Workers (MSWs) Social Work Assistants, work in close collaboration with the doctors, nurses, and therapists to provide the necessary social and comprehensive patient-centered care to our patients.
The MSWs attend to the emotional, psychological, social and care needs of our patients to help them cope more effectively with the challenges arising from various medical conditions.
Our services include:
Information, liaising and referral to community care services.
Financial assistance.
Care and discharge planning.
Psychosocial support
Group work and psychoeducation for patients and caregivers.
