What is Smart Ageing


Smart ageing refers to the use of technology and innovative approaches to enhance the quality of life for seniors. As our population ages, there is a growing need to address the unique challenges faced by our seniors, such as healthcare, social isolation and their overall wellbeing.


In response to this, SCH has been constantly developing and implementing smart ageing initiatives to enhance the care provided to our elderly patients, aiming to restore them to optimal health and wellbeing. One of our smart ageing approach is our internationally renowned care model that incorporates Social Prescribing into our holistic care plan for patients.

In this programme, our Wellbeing Coordinators (WBCs) will assess patients' social determinants of health and co-design a social prescription that compliments the medical prescription, with the aim of improving their health and quality of life.  This is personalised to what is important and meaningful to our patients and involve activities related to arts, nature, heritage and physical exercise.  Upon discharge, they are linked to community partners to continue their social prescription.

Driving these efforts is none other than Associate Professor Lee Kheng Hock, SCH's Deputy CEO (Education and Community Partnerships) and the team at OCEAN (Office of Community Engagement and Education). 




Our programme is well known internationally and in recognition of his contributions to Smart Ageing, he was recently awarded the prestigious Smart Ageing Leader Award conferred by the Golden Age Foundation in Hong Kong SAR. 

This was presented to Prof Lee by Mr Chris Sun Yuk-han, Secretary for Labour and Welfare in Hong Kong, during the Smart Ageing Leader Award Presentation Gala Dinner as part of the 9 th Golden Age Expo and Summit.

This prestigious event commends outstanding leaders who have inspired and encouraged professionals and entrepreneurs to develop innovative initiatives that help reshape the future of smart ageing. The accolade is a true reflection of Prof Lee's outstanding contributions towards patient care and SCH's international reputation as an exemplar of  social prescribing and smart ageing.
