Assoc Prof Lee Kheng Hock
In 2006, Assoc Prof Lee set up the first hospital-based family medicine department in Singapore, providing both inpatient and transitional care services at the Singapore General Hospital. He is a pioneer in the field of integrated care, working towards creating a sustainable healthcare system for a rapidly ageing population. His work is validated by two landmark randomised control studies that proved the effectiveness of transitional care services in reducing unnecessary hospital utilisation.Assoc Prof Lee teaches Family Medicine at the Duke-NUS Medical School Singapore. He incorporated patient-centred care and care integration into the Family Medicine curriculum. As the Director of Education of SingHealth Community Hospitals, he oversees clinical training of three community hospitals.
