About SingHealth Community Hospitals

SingHealth Community Hospitals (SCH) comprises of Sengkang Community Hospital and Outram Community Hospital. Being Singapore's only cluster of community hospitals allows us to shape the way we deliver person-centred care to patients. 

Our community hospitals are poised to set standards in care, exchange best practices and strengthen collaborations with our healthcare and community partners.

We provide medical, nursing and rehabilitation services, with emphasis on rehabilitation to get the patients well enough to return to the community.

At SCH, our focus goes beyond healthcare to health. To promote healing, our community hospitals provide a homely environment for patients to recuperate and take part in well-paced personalised care. 

We want our patients to not only recover from their medical conditions but also to return to their family and be supported by a helpful and connected community. Palliative patients will also receive thoughtful, compassionate and dignified care.


A Member of SingHealth Regional Health System

SCH is under the SingHealth Regional Health System (RHS) and is supported by the medical expertise and research capabilities of the SingHealth-Duke NUS Academic Medical Centre. 

Teaching Hospital

SCH is a teaching hospital for undergraduate medical training as well as postgraduate and advanced training of specialist doctors, nurses and allied health professionals. 

Students from both the National University of Singapore’s Yong Loo Lin School of Medicine, Duke-NUS Medical School and Lee Kong Chian School of Medicine spend a substantial part of their curriculum in SCH. The hospital’s medical staff hold teaching positions at both schools. 

It also serves as a clinical teaching hospital for student nurses and therapists from the polytechnics and medical institutions here and the Asia Pacific region.

We aim to deliver a seamless and integrated care path for patients requiring sub-acute care, rehabilitation and palliative care

Beyond Hospital to Community

Enhanced patient flow - We will work closely with SGH, SKH and CGH to proactively identify patients suitable for transfer. This collaboration will allow us to facilitate patients' move to or from our community hospitals, simplify referral processes, and extend transfer cut-off time. 

Better post-discharge care and support - To ensure that patients are cared for even after discharge, we will collaborate with community partners through initiatives like joint discharge planning, developing care protocols and a training centre for our partners.
