​Preparing For Admission

Pre-admission Procedure

Inpatient Services

  • Admission to SCH is only by doctor's referral from a registered acute hospital.
  • Appointment for admission is made by prior arrangement with SCH.

 

Outpatient & Home Care Services - only applicable to Bright Vision Community Hospital (BVCH)

  • Registration is by appointment.
  • Pre-registration by mail or fax for specific education programmes is welcomed.

Documents to bring

Please bring along the original documents of the following to register for your admission:

  • NRIC or Permanent Resident document / passport
  • Medical reports
  • Laboratory / X-ray results
  • Any other medical health reports available
  • Referral letter
  • Civil Servant Card
  • Medical Exemption / Medical Fee Assistance / Public Assistance Card

If you have the following documents, please bring them along:

  • Letter of Guarantee from Employer
  • Medical Claim Authorisation Form (MCAF)
  • Means-Test Declaration Form
  • Medisave Account Holder's NRIC

Our Business Liaison Officers will provide you with information on the available financing schemes to help you make an informed decision on your ward type. 

 

Deposit

For non-subsidised patients, a deposit is required upon admission. The deposit amount varies with the type of services required.


Cancellation

Please notify the Business Office of any cancellation prior to your admission.
