Registration Terms and Conditions

PDPA

I consent to Singapore General Hospital and its related corporations (collectively 'SingHealth), their agents and SingHealth's authorised service providers collecting, using, disclosing and/or processing my personal data, in order to send me marketing and publicity materials.

I confirm and agree that my consents granted herein do not supersede or replace any other consents which I may have previously provided to SingHealth in respect of my personal data, and are additional to any rights which SingHealth may have at law to collect, use or disclose my personal data.

By providing the information set out in this form and submitting the same to you, I confirm that I have read, understood and consent to the SingHealth Data Protection Policy, a copy of which is available at http://www.singhealth.com.sg/pdpa.

PAYMENT

All payment must be made in Singapore Dollars (SGD) and to be received by 15 July 2018. The above stated registration fees are inclusive of 7% GST.

Due to limited places, registration will only be confirmed via email upon receipt of full payment on a first-come-first served basis. The following payments methods are accepted:

a) Credit Cards (Master/VISA)

Please fill up the credit card authorisation form and email the completed and signed form to entcourses@sgh.com.sg

 

b) Cheque/Bank Draft

Each cheque/bank draft must be indicated with the delegate's name, contact number and email address to accelerate the registration process and to ensure payment is allocated to the correct registration/course. Cheque/bank draft must be crossed and made payable to "Singapore General Hospital Pte Ltd".

b) Telegraphic Transfer/Bank Transfer

For telegraphic or bank transfer, please contact entcourses@sgh.com.sg for bank and swift code details.

Kindly ensure that payment made by telegraphic transfer/bank transfer is NETT of all bank charges and commissions. All administrative and bank charges are to be borne by the delegate. Please indicate delegate name and email address on the remittance advice. Upon completion of the transfer, please email a copy of your remittance advice with delegate name (as per registration) and contact number to the Conference Secretariat at entcourses@sgh.com.sg for verification.

CANCELLATION POLICY

There will be no refund of registration fee for cancellations made after registration and for non-attendance

The Organiser reserves the right to modify the programme. The Organiser reserves the right to cancel the Conference and fully refund the participants less administrative and bank charges should unforeseen circumstances necessitate it.

REPLACEMENT POLICY

Participants unable to attend the Conference may send an alternate, i.e. single substitution only. Substitution requests must be submitted via email to the Conference Secretariat at entcourses@sgh.com.sg by 15 July 2018.

LIABILITY AND DISCLAIMER

Whilst every attempt will be made to ensure that all aspects of the event mentioned herein will take place as scheduled, ENT Fortnight 2018 Organisers reserve the prerogative to make final changes should the need arise. The applicant acknowledges that he/she has no right to lodge complaints against ENT Fortnight 2018 Conference Organisers should the holding of the event be hindered or prevented by unexpected political or economic events, or by force majeure, non-appearance of speakers or other reasons which necessitate programme changes. With registration, the applicant accepts this Provision.
