The Healthier SG strategy shifts the emphasis of care from reactively caring for those who are already sick, to proactively preventing individuals from falling ill. As population health manager for the Eastern region, SingHealth recognises that we need to partner community activators in the social service sector to make a meaningful impact to population health.
Developing an effective and holistic population health approach also requires us to look at the family as a single unit, and integrate the health and social care elements. In this forum, we will explore the challenges that families face, and the social determinants of health affecting them.
We will discuss how care partners can come together to take a family-centric approach, and learn about the roles of each contributor in the SingHealth Healthier SG Team.
