Seamless care for patients with Parkinson Disease

​Parkinson Disease (PD) is the second most common neurodegenerative disease in Singapore, affecting three in every 1,000 people aged 50 and above.  It is estimated that there are around 8,000 patients with PD in Singapore. These patients require long-term treatment but their movements are often limited with this debilitating condition. Many of them would therefore benefit from community and home-based care.

 

Community Care Partners Programme

Launched in 2007 by the National Neuroscience Institute (NNI), the Community Care Partners Programme (CCPP) aims to enhance community care and support to PD patients by training community care providers.

To date, more than 400 nurses and allied health professionals from 27 ILTC and voluntary welfare organisations have been trained to manage the unique needs of PD patients. The programme includes:

  • Monthly rounds and interactive case discussions with the multidisciplinary care team at NNI
  • Workshops and update sessions every four to six months
  • Annual refresher courses and training workshops

Home Care Training and Support Programme

The programme aims to empower home care nurses who care for PD patients who are home-bound, bedridden or with poor social support. The programme comprises:

  • Home care training: Home care nurses will undergo on-the-job training through clinic and home visit attachments, multidisciplinary rounds at NNI, and e-learning.

  • Home care support: Nurses will conduct home visits for at-risk PD patients (Stage 3 and above according to the Hoehn and Yahr scale) and provide caregiver training.

Patients who need additional support will be referred to allied health professionals and community partners to ensure continuity of care.

To date, more than 30 home care nurses and caregivers have participated in the training.
