Dr Adrian Tan Kok Heng

Dr Tan Kok Heng Adrian is a Family Physician Consultant in the Department of Continuing and Community Care(CCC) in Tan Tock Seng Hospital(TTSH). He is currently Deputy Director of Home Ventilation Respiratory Support Service, the Consultant in charge of Transition Care Teams in CCC and consultancy work with Ang Mo Kio and Renci Community Hospital, and he is a mentor in Advance Care Plan in Tan Tock Seng Hospital.

He obtained his MBBS (NUS) in 1997 followed by MMed family medicine NUS in 2003 . He is a fellow of College of Family Physicians (FCFP) since 2010. He also obtained his geriatric diploma GDGM in 2007.He was award the Health Manpower Development Plan on High Spinal Cord Injury Patient Management in 2014 to Shepherd Centre, Rehabilitation Centre Altanta, Georgia USA.

He is currently appointed Adj Assistant Professor of the Medical Department, Family Medicine, National University Health System. He is also a faculty member of the Family Medicine Residency Program of National Healthcare Group and Clinical practice Facilitator of Lee Kong Chian School of Medicine.

His current interest includes community complex care, geriatric, respiratory, rehabilitation and palliative care.
