Designed by curriculum specialists and subject matter experts in the Education (Social Service) and Healthcare fields, the 4-module Health and Social Care Coordinator (HSCC) course aims to equip learners with abilities needed to perform tasks in an emerging field of work namely in the health and social care sectors. Learners will attain skills in working with multi-disciplinary teams of healthcare professionals and co-creating social care plans for elderly patients.
Job roles that participants of this course can potentially look forward to include: Care Coordinator, Wellbeing Coordinator, Health & Social Care Coordinator, Care Coordinator Associate, Programme Coordinator (Volunteer/ Befriender Management), Programme Associate (Senior Activity Centre), Activities Coordinator, Care Programme Coordinator, and Care Associate. This course is offered under the SGUnited Skills programme and will be jointly delivered by SingHealth Community Hospitals’ Office of Learning (SCHOOL) and Ngee Ann Polytechnic’s CET Academy.
