Health Management Programmes (HMPs)

The Health Management Programmes (HMPs) have been developed as part of SingHealth’s Clinical quality initiatives. We aim to ensure high standards of care through a comprehensive disease management approach, utilising structured work processes and evidence-based medicine to enhance clinical practice and patient safety across the continuum of care. Concurrently, efficient resource utilisation is an important consideration.
